IMPORTANT! Dogs are eligible for a Natural Ability Test up until, and including, the day they reach 16 months of age. Dogs over 16 months may be tested for evaluation. No Prize Classifications are awarded dogs tested for evaluation. Dogs over 16 months may be tested if space is available.

This application filled out legibly and completely.
Copy of registration certificate, litter papers or a written explanation why proof of registrablity is not available.
Entry fee in the form of check or money order, made payable in US Funds to the NAVHDA - Rappahannock Chapter.
Copy of hip dysplasia X-Ray evaluation (if applicable.)
