Without completion of this application, your dog will not be included in the Foundation Stock Stud Book.

In order for your Spinone to be included in the initial foundation stock for the AKC Stud Book you must do the following:

• Complete ALL information on this Application
• Sign and Date the Application
• Include a check made payable to Spinone Registration in the amount of $10.00.
• Mail completed, signed application and remittance to:
